Quality

This page is for coordinators, registered managers and owners. It covers the two tables on the quality report, what each column means, and what the single export contains.

“Quality” is described as “Missed visits and the incident trend: the Safe and Effective lines of enquiry.”

It is the report to have open when someone asks what went wrong and whether it is getting better or worse.

The presets are “Last 4 weeks”, “Last 12 weeks”, “This month” and “Custom”. It opens on “Last 4 weeks”. “Custom” reveals “From” and “To”. The dates covered are printed under the heading.

The first card is “Missed visits”, described as “Published visits that ended with nobody checked in.”

A visit qualifies when all of these are true: it was published, its end time has passed, and nobody on it checked in. A visit still inside its window is running late, not missed, and belongs to the alerts inbox instead.

Column What it means
“When” The date and the planned times. It opens the visit.
“Client” The person who did not get the visit. It opens their record on the “Timeline” tab.
“Assigned” Who was meant to be there, or “Nobody was assigned” when the visit had no one on it.

A visit with nobody assigned is the worst kind of missed visit, because nobody was ever going to arrive.

The search box narrows this table by client name and by the names of the people assigned. It does not narrow the incident trend.

When there is nothing, the card says “No missed visits in this period.”

The second card is “Incident trend”, described as “Incidents by week, severity and kind.” Its description line carries the period’s summary in the form “8 low · 3 medium · 1 high · 1 CQC-notifiable · 1 notified to CQC · 0 safeguarding referrals”.

Column What it means
“Week of” The first day of the week, on your organisation’s week start. It opens the incident log filtered to that week.
“Low”, “Medium”, “High” Incidents in that week at each severity. High is shown in red.
“Total” All incidents that week.

Under the table is a line of counts by category: fall, medication error, safeguarding concern and the rest.

Incidents are counted by the date they happened, not the date they were reported. No client is named on this card.

When there is nothing, the card says “No incidents in this period.”

One button, one file. An inspector asks for the quality report for the quarter, not for two downloads to staple together.

The file holds three sections, separated by blank rows:

  1. Missed visits: date, starts_at, ends_at, client, assigned
  2. Incident trend: week, low, medium, high, total
  3. Incidents by category: category, count, then cqc_notifiable, cqc_notified and safeguarding_referred rows

The missed visits section carries exactly the rows on screen after your search. The file name carries the dates of the period.
